As a result of the coronavirus (COVID-19) pandemic we are asking all our sit-in customers for some basic contact details in order to keep everyone safe. In order to provide the safest environment possible please could you fill out the attached form (BEFORE ORDERING).

By maintaining records of staff, customers and visitors, and sharing these with NHS Test and Trace where requested, you can help us to identify people who may have been exposed to the virus.

Your information will be stored securely for 21 days and then be deleted. It will be not be used for any other purpose other than contract tracing a potential coronavirus outbreak.
